We are using the QODBC driver to integrate quickbooks.
After setting up a new customer, we have a problem creating records.
When we try to insert a customer record, we get an error saying the
customer already exists but they do not.
The program works for other clients so I expect it's a QODBC install/configuration issue.

As per How do I add customers? , Name must be unique across Customer, Employee, Vendor, and OtherName. You can query the Entity to verify if a name already exists.
